We are currently seeking a qualified individual to assume a leadership role in the development of automated curriculum content and the oversight and implementation of live coaching content and protocols for its text messaging-based health engagement and behavior change programs.
A qualified candidate will be a seasoned certified health coach, preferably a Certified Diabetes Educator (CDE) and/or RN, with direct experience in the development, implementation and delivery of intervention programs for condition management, transition of care (post hospital discharge management) and/or wellness/health improvement, provided via telephonic, digital and/or in-person modalities. Prior experience in developing and implementing digital and/or mobile (mHealth) interventions is desirable, genuine interest in leveraging mobile and digital technology to transform and improve the way guidance and support is provided to people who need to change their behavior and improve their health is mandatory. The successful candidate will have strong leadership and interpersonal skills, will be able to function as an effective team member, and be highly motivated, energetic, versatile, and passionate about making a difference.
The Clinical Program Manager will:
Assume primary responsibility for the creation and maintenance of curriculum content (automated and live messaging, as well as associated web-based content/resources) for all new and existing programs including: –Designing and developing curriculum content for new programs –Systematically and periodically reviewing and updating existing program content –Ensuring that evidence-based clinical guidelines and behavioral science are applied appropriately and consistently to create and implement curriculum content for new programs and modifications to existing programs.
Establish success criteria for all programs and leverage data to drive informed decisions for evolving existing programs and client outcomes
Partner with the Company’s behavioral scientist and executive leadership team to ensure that the Company’s behavioral intervention framework (model, methodologies, tools and techniques) is properly documented and maintained, and evolves appropriately with evidence-based guidelines and best practices over time
Develop and implement standard content and protocols for the delivery of live coaching guidance and support to program participants
Train, oversee and serve as a clinical resource to health coaches when assessing and formulating intervention/escalation strategies for participants in need of additional counseling/support.
Work with client and Company product personnel as appropriate to adapt/modify existing curriculum content and/or create customized or substitute alternative content as required
Work with client personnel to establish and implement escalation protocols for the referral of participants with clinical, medical and/or medication related questions/concerns to the client care team
Serve as clinical program subject matter expert in support of sales and account management
• Bachelor’s degree or higher in nursing, health promotion, health education, public health or other health related field • At least 5 years of experience as a health coach/counselor (direct experience with diabetes prevention and management, as well as cardiac condition management and general heart health are preferred) • Diabetes Educator Certification required • Following certifications are preferred: -Certified Wellcoach or similar health coaching certification -Certified Diabetes Prevention Lifestyle Coach -Certified Tobacco Cessation coach -Other chronic condition coaching certification • Experience in the development and implementation of intervention programs for condition management, transition of care (post hospital discharge management) and/or wellness/health improvement, provided via either telephonic, digital or in-person modalities mandatory • Direct, hands on experience in authoring consumer-facing wellness and clinical education program material based on credible sources and evidence-based guidelines • Prior experience in developing and implementing digital and/or mobile (mHealth) interventions is desirable • Practical knowledge of behavioral modification and/or health coaching methodologies/techniques • Knowledge with data and analytics • Proven experience in crafting succinct messaging designed to engage, motivate and encourage individuals to take an active role in living a healthier lifestyle • Project management skills; ability to establish timelines/project plans and adhere to schedules • Strong analytical and problem-solving skills • Strong leadership and interpersonal skills; ability to facilitate meetings with both internal and external customers via both conference call and in person • Detail oriented individual with ability to determine root cause and assess corrective action • Proficient in Microsoft Office including, Word, Excel, PowerPoint and Project • Excellent organizational, planning, and prioritization skills • Must be able to travel as needed
