JOB SUMMARY: Works under clinical supervision of the Medical Director and reports administratively to the Director of Home Based Programs.
Serves in a complex and expanded nursing role to collaborate with members of the Home-Based Care Team, which includes primary care provider, social worker, and other care team providers to provide intense case management and clinical interventions that ensures the delivery of quality, efficient, and cost-effective healthcare services specifically related to complex patient in the home setting.
Responsible for the management and oversight of a caseload consisting of high risk patients with complex medical/psychosocial needs. Assesses, plans, implements, coordinates, monitors and evaluates the care plan and deploys the necessary interventions with the goal of optimizing the patient’s state of well-being. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the most complex patients.
Identifies health problems and provides restorative, supportive, preventive and specialized clinical nursing care to individuals in their home setting and executes clinical medical regimens as prescribed by the licensed provider overseeing care. Fully responsible for all actions as a licensed nurse and is accountable to patients for the coordination and delivery of quality of care.

EDUCATION AND/OR EXPERIENCE: RN license or eligible for licensure in the state of Pennsylvania required. Bachelor's degree in nursing preferred.
Three years’ experience in clinical nursing, critical care preferred. Case Management experience preferred.
IV skills, venipuncture required.
Critical thinking skills and ability to analyze complex data sets required. General computer knowledge and capability to use computers required.
Obtain Case Management Certification within 2 years required.
WORKING CONDITIONS/PHYSICAL DEMANDS: On call required Evening and weekend rotation Work is typically performed in a patient-home based environment. Duties involve occupational exposure to disease and illness including Requires HBV vaccinations and TB skin test
