The VA Maryland Health Care System (VAMHCS) is a Level I tertiary multi campus teaching facility affiliated with the University of Maryland, School of Medicine and over 100 educational institutions. The VAMHCS consists of the acute care Baltimore facility; the multi building Perry Point campus ; the Loch Raven campus ; and six urban and rural outpatient clinics. The Patient Aligned Care Team (PACT) LPN serves as the essential role of bringing the patient into the exam room for the primary care visit. He/she is part of the standard PACT Teamlet model at the VAMHCS. The LPN checks the patient's vital signs, weighs the patient, interviews the patient using the standardized nursing note template and VHA's clinical reminders. The LPN communicates new patient information to the provider. He/she administers vaccinations and other injections, performs all point-of-care ancillary lab testing for glucose, Hemoglobin A1C, PT/INR, and performs EKG's. The PACT LPN supports the patient's health promotion through follow-up teaching on hypertension, diabetes, and other co-morbidities. In addition, they participates in the daily plan of care for the Veteran, daily PACT Teamlets huddles and weekly huddles. Work Schedule: Monday - Friday 0800 - 1730
