The PACT RN is a registered professional nurse who is accountable for the provision of client care at the Finger Lakes Healthcare System. The Registered Nurse is directly responsible to the Nurse Manager of their specific service. The work location for this position is the Elmira Community Based Outpatient Clinic (CBOC); however, occasional travel may be required to other clinics in the Finger Lakes Healthcare System. The PACT RN uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic are plan for life. The PACT RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The PACT RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The PACT RN demonstrates leadership by serving as an advocate for patients, team player or colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Meets all mandatory requirements for assigned area and will perform activities that reflect the educational, experiential, and competency requirements outlined in the 4 Dimensions and the criteria identified under each Dimension. The RN performs, reports, and records assigned nursing care in the Primary Care Clinics as described below and any other duties assigned by management. a. Triages walk-ins appropriately. b. Uses appropriate telephone etiquette for both internal and external customers. c. Uses telephone assessment skills to identify actual and/or potential health, safety, and educational needs of the patient. d. Appropriately uses triage decision support based from the Telephone Triage Protocols for Nurses by Briggs to systematically assess and address caller needs. e. Appropriately prioritizes the urgency of caller needs utilizing critical thinking skills and decision support tools. f. Develops a collaborative plan of care with caller, their support systems, and their provider. g. Completes documentation for clinical calls/TRM's within 24 hours of call. h. Enters as ordered, locates as needed, and obtains results of x-rays, tests, procedures, labs. i. Engages and documents patient education. j. Incorporates knowledge of ethical/legal issues and professional nursing standards necessary for providing patient care and information. k. Knowledge of and implementation of PACT principles. l. Completes hospital discharge follow-up calls within two business days and UCC/ER visit calls. m. Documents hospital discharge and post discharge UCC/ER calls in CPRS. n. Utilizes secure message as a means of communication with patients and documents pertinent health information in CPRS. o. Facilitates and acts a provider in RN clinic including providing knowledge/information to patient/family to promote self care of acute/chronic disease management. p. Applies clinical knowledge of primary health care in prevention and wellness, health risk assessment, disease management, and self care. q. Adheres to proper infection control procedures regarding equipment, cleaning and storage. r. Understands responsibility for the safe processing of reusable medical equipment. s. Monitors exam rooms and clinic frequently for cleanliness and patient presence. t. Follows policies for insuring the correct patient is receiving the correct treatment by using two patient identifiers. u. Performs procedures as directed, i.e. EKG, Doppler, I&O catheterization, venipuncture, PPD application, nebulizer treatments, dressing changes, vital signs and weights, etc. v. Administers medications as ordered and documents appropriately. w. Documents clinical reminders. x. Adheres to Missed Opportunities Policy for cancellation of provider/Med RN appointments. y. Follows policies/procedures developed by the service, facility, and VISN. z. Facilitates flow of patients through the clinic as team facilitator by assigning tasks/roles to RN's, LPN's, Health Technicians, and MSA's.
