The Community Engagement Director serves as a resource for reliable information and to create dynamic and lasting relationships between our health system and the communities we serve.
Responsibilities:
- Serves as Bon Secours Charity Health System (BSCHS) community representative and arrange for hospital participation in community events throughout the year.
- Develops and implements community programs in response to hospital-specific Community Health Needs Assessments (CHNA) and community health initiatives; partner with local and state health departments and other community service providers to maximize efforts.
- Plans, directs, and evaluates full spectrum of community initiatives, including health education programs, support groups, and CHNA and NYS Community Service Plan (CSP) related events.
- Ensures that specific community education programs are offered to fulfill American College of Surgeons' requirements for GSH Cancer Program and NAPBC Breast Center accreditations.
- Plans bi-annual Stroke Awareness community programs as per JCAHO and NYSDOH Stroke Center accreditation standards (GSH).
- Develops and maintains relationships with community members and organizations to promote BSCHS as the local expert for healthcare and health education.
- Tracks and evaluates effectiveness and appropriateness of outreach activities.
- Manages distribution of information to the public and internal stakeholders as it relates to community engagement services.
- Works closely with BSCHS/WMCHealth executive leadership to support the Network Strategic Plan as it relates to community initiatives.
- Maintains current listing of all presenters and recruit new presenters from among WMCHealth physicians, leadership and program directors and staff.
- Matches community requests with appropriate speakers with particular emphasis on BSCHS employed physicians and staff.
- Proactively seeks opportunities to connect new BSCHS physicians as speakers with local audiences.
- Captures data as necessary for Community Benefit reporting.
- Ensures community education/prevention events are planned and executed to fulfill accreditation requirements of The Joint Commission, American College of Surgeons, NYSDOH, and the NAPBC for the following hospital programs:
- Cardiac surgery (GSH)
- Stroke Center (GSH)
- Cancer Program (GSH)
- Center for Breast Health
- Works in partnership with local health departments (LHD) to perform a community needs assessment and develop a Combined Community Health Improvement Plan (CHIP)/CSP for each county.
- Assists LHD with need prioritization and development of joint action plans (in partnership with other hospitals within the same county).
- Leads hospital teams to fulfill deliverables as outlined in CHIP/CSP; including assisting hospital staff to ensure the agreed upon action plan deliverables are met.
- Submits yearly updates to LHD and NYS.
- Represents BSCHS with community-based organizations with the focus being community engagement and wellness.
- Represents BSCHS to local health departments and support their efforts.
- Performs other duties as assigned
Qualifications/Requirements:
5 years of healthcare experience, or in a position in which community relations or public outreach, or in working for New York State (or one of its agencies or subsidiaries) or a local government in the State of New York required.A Valid Driver's License required. Flexible schedule and travel is a must; availability for weekdays, weekends, early mornings, late evenings, etc.
EDUCATION:
Bachelor's Degree or higher in a healthcare related field required.
OTHER:
Must be aservant leader with Excellent planning, communication and record keeping skills. Great attention to details required as the ultimate face of the health system for community events.
