Under the direction of providers (defined as an MD, DO, DPM, NP, or PA) or RNs, the LVN/LPN provides clinical support functions and professional nursing care to patients using established standards of clinical nursing care and procedures approved by the applicable state board of nursing. Is an active member of the health care team to assess patients’ needs, provide input into the plan of care and implement prescribed interventions.
Primary Responsibilities:
Performs all nursingduties within thescopeofaLicensed Vocational/Practical Nurse (procedures, injections,medication administration, EKGs, phlebotomy)asdetermined bythe applicableStateBoard ofNursing
Participates in implementingestablishededucationplans forpatients and their families with common health problemsand well-definedhealth learningneeds
Coordinates patient careas directed byproviders,RNs, and policies, includingreferrals, transition visits, medications, procedures, and follow-up
Documents chiefcomplaint and rooms patients accordingto policyand procedures, prepares patient for examination
Telephonicallyobtains information on chiefcomplaint and symptoms and accuratelyrelays information to the provider
Collects data and performs “focused”nursingassessment ofpatients; assists in the evaluation ofapatient’s responsetotreatment and identifiespatient’s needs; communicates findings to theprovider
Records patient assessment and carein themedical record in anaccurateand timelymanner
Reviews thepatient record, chart, reports (includinglaboratoryand x-ray), and other pertinent information forpatients priorto beingseen bytheproviderand reports relevant information to theprovider and/orRN
Provides asafeenvironment (OSHAstandards) forpatients and staffin theclinic
Ensures patient confidentiality(HIPAA) at all times and treats patients with courtesyand respect
Organizes exam and treatment rooms, stocks and cleans rooms and sterilizes instruments
Practices standard infection control precautions
Ensures medication management follows policiesand procedures; appropriatelydocuments medications &immunizations in patient records
Follows laboratorymanagement policies &procedures and CLIAstandards; collects lab samples and ensures properlabeling

Required Qualifications:
High school diplomaorGED
Current and unrestrictedState Vocational/Practical Nursinglicense
Current BLS certification
Ability to obtain IV certification ifrequired by the clinic
Knowledgeofmedical terminologyand medical officeprocedures
Proficient computerskills to work efficientlywithelectronicmedical records
Proven abilityto perform medical procedures,giveinjections and perform phlebotomy
Proven abilitytoestablish andmaintain effective workingrelationship with providers/staff, patients, and the community
Proven good communication and customerserviceskills
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
