The Pediatric Specialty Clinic - Woodbury is a free-standing clinic that offers a wide range of services to pediatric patients seeking care from any of our several sub-specialties. Some of the pediatric sub-specialties we see include; Neurology, Cardiology, Endocrine, Diabetes, Gastroenterology, Rheumatology, Weight Management, Sleep medicine, Pulmonology, Surgery, NICU Follow-Up, Nephrology, Dermatology and Urology. Many of our clinics are multi-disciplinary, which means we offer multiple specialty services within one clinic appointment. We are a supportive, cohesive team that is passionate about pediatrics.

What you will do as an RN Care Coordinator:

• Coordinates care across the health care continuum within designated sub-specialty population(s)
• Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services
• Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits
• Facilitates proactive disease management and preventative health care using evidence-based practice
• Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:

• Current Minnesota RN license
• 3+ years of Clinical Nursing experience
• BLS certification
• Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy

Hours: .7 FTE, 30 hours a week
